In short, Choice Pass is: a FREE program for 6-12th graders of the Gunnison Valley and Lake City that celebrates their healthy choices through discounts to nearly 60 local businesses and numerous free or discounted programs. For more information regarding requirements click here.

Necessary Paperwork

Teen survey, parent survey, and registration need to be completed before teens and parents attend a Sign-Up Night.

1. Each teen signing up for Choice Pass, newbies and returners alike, need to complete the Teen Survey. They can link this link or use the QR Code. Parents- please grant your kids privacy to fill out the survey honestly.

2. ***At the bottom of the Teen Survey, in bolded red letters, there is a link to the registration page. Teens need to hand their device to their parents to complete the registration.***

3. A parent/guardian, newbies and returners alike, need to complete the Parent Survey. Parents can take the survey via this link or by using the QR code. Parents only need to take the survey ONCE, regardless of the number of children they have.
