Registration 2026-2027

A LAST CHANCE Sign-Up Night will be held Monday, October 19th at 5pm at the Ann Zugelder Building (307 N. Wisconsin St- in Gunnison). Doors will close at 5:10- no exceptions.

*PARENTS/GUARDIANS AND YOUTH are both REQUIRED to attend a Choice Pass Sign-Up night. Failure to attend, by either party, will result in the youth being ineligible for Choice Pass in 2026-27.

Follow Steps 1-4

1.  Youth: complete the online survey. If you have more than one 6th-12th grader, they EACH need to complete the survey.

Note to parents: Please have your youth fill this out independently, without parental help. Thanks!

2.  Complete the registration/waiver-- the link for the waiver is provided at the end of the youth survey.

3. Parent: complete the online parent survey. If you have multiple kids, you only need to take this survey once.

4. At sign-up night, you will be given a copy of the Choice Pass Commitment. This is an opportunity as a family to set clear boundaries and expectations around drugs, nicotine, and alcohol. It’s also an opportunity to think of ways that you, as the parent/guardian, can support your youth’s choices, even when they make choices that you disagree with and even go against their Choice Pass Commitment. We encourage you to take time each year to reflect on your youth’s development and make thoughtful changes in how you might respond to a breach of the Choice Pass Commitment.

NEW FAMILIES:

New families and families of 6th graders, are encouraged to attend sign-up night on either Monday, September 14th at Fred Field Center in Gunnison or Monday, September 22nd at CBCS in the Multi-Purpose Room (MPR). These nights are geared towards those who are entering Choice Pass for the first time, though families can attend any of the sign-up nights.
